Payment Policy

We accept debit, credit, cash, or check.

You can call and pay by phone with your debit/credit card, you can mail us a check, or you can stop by our clinic any time between normal business hours and pay any way your perfer.

Self Pay Patients

Our office visit charge for self pay patients is $50. Labs and injections are not included in this fee and are an extra charge.

We offer a sliding fee scale for our self pay patients. This scale is based off of income. This scale will only decrease the pay of the office visit and injections. Labs are not included in the sliding fee scale. Please click here for further information on the sliding fee scale.
